Custom ignore rules (replaces "Ignore dates")

• The single Ignore dates toggle is now an editable Ignore phrases list. Each rule pairs a label with a regular expression matched against the whole phrase name; invalid regexes are flagged and skipped.
• Default rules: Numbers ^\d+$ and Dates ^\d[\d\s\-/:.,+TZtz]*$ (keeps the old date-toggle behavior).
• Rules now also gate existing-note linking — fixes stray [[6]] links that appeared when a numeric note name got indexed (e.g. from an accidental [[6]] in another note): plain numbers no longer link to a note named 6.
• Old ignoreDates setting auto-migrates on load (true/default keeps the Dates rule, false drops it).

Bug fixes

• Existing-note linking no longer touches URLs: a note named after a URL host (e.g. Budget) never turns https://budget.example.dev/ into https://[[Budget]].example.dev/, and other URL matches are skipped.

Tests

• 226 unit tests, all passing.

• Proximity-based ranking — the preview ranks repeated-phrase suggestions by how closely their occurrences cluster in the document.
• Progress bar on vault scans — real done/total progress in the scanning modal; now cancelable.
