fix: replace StackWalk64 with RtlVirtualUnwind for stack traces

StackWalk64 depends on SymFunctionTableAccess64 (dbghelp) which fails
to unwind through modules without PDB symbols. Use OS-native
RtlLookupFunctionEntry + RtlVirtualUnwind on x64 (reads .pdata
directly), and EBP chain walking on x86.
