Graph v1.0.4

Graph starts progress only after create_goal_graph successfully creates the graph. A slash command that produces only a plan no longer publishes an empty running projection or overwrites an existing branch's progress.

Requires an OpenAgent Runtime implementing the current generic plugin contract and Host Bridge.

Validation: 5 package tests passed; plugin-kit manifest and component validation passed. The attached ZIP contains plugin.json at the archive root and preserves package identity. Existing plugin data remains managed by OpenAgent during an explicit update.
