Feeds agent-action command stdout, stderr, exit code, and errors back into the VS Code chat workflow. Normal chat now automatically continues after command execution with tool results, and non-zero command exits preserve output instead of breaking the workflow. Refreshes EXE/source packages as v0.4.2.
