v3.14.3 — finish removing prices from the docs

Completes what 3.14.2 started. That pass swept the tables and doc comments but missed prices living inside README code blocks — 14 Polymarket and Exa examples annotated with ($0.001/request), ($0.005/request — tier 2) and the like — plus the FAQ's "Prices start at $0.0002 per request" and an integration-test cost estimate.

Tier labels stay: a tier is a stable classification, not a rate.

The whole file was audited rather than pattern-matched again. Every dollar figure left in README.md is a $0 free-tier statement, the "$5 covers thousands of requests" framing, wallet funding guidance, or a sample balance in example output.

Live rates: blockrun.ai/models, or client.listModels() / client.listImageModels() at runtime.

No API or behaviour change.
