Fix: use IP-based unique IDs for all sensors

  • All panel sensors now use a stable name based on source + IP + panel ID + parameter.
  • Prevents sensor swapping after Home Assistant restart.
  • Requires re-adding the integration and updating any automations referencing old sensor names.
