Fix confusing output labels and broken mindmap rendering.

Output pills renamed: "Summary"/"Short" → "Detailed"/"Concise" (no more overlap with Length selector)
"Output" label → "Format"
Fixed markmap CDN: markmap-lib@0.17 browser bundle was returning 404, updated to 0.18
