Release v0.12.0

Portkey control-plane compatibility and runtime-hardening release. The catalog
grows from 178 to 181 tools, so MCP clients should refresh tools/list.

Added

• Add list, complete replacement, and single-server upsert tools for guardrail
  MCP-server mappings, plus the documented llm and mcp_tools guardrail
  targets and their target-specific validation.
• Add deployment tag filtering and create/update support, including distinct
  omitted, replacement-map, and explicit-clear update semantics.
• Expand usage-policy updates with description, replacement conditions,
  periodic reset days, and nullable next-reset timestamps without widening the
  create contract.
• Publish a maintained distribution inventory covering owner-controlled MCP
  records, stale downstream mirrors, the Awesome MCP Servers submission, and
  the Docker Catalog and Smithery decisions.

Changed

• Mark external MCP server instructions as untrusted self-reported result data
  with explicit provenance while preserving the upstream value for review.
• Move Renovate to dev/0.12; update the Redis CI image, pinned workflow
  actions, development toolchain, Biome/Qlty alignment, and both Docker stages
  to Node.js 24.20.0.
• Exercise LobeHub generation behavior against all 181 runtime tools, including
  paginated discovery and refusal to publish a shrinking catalog.

Fixed

• Keep outbound request deadlines active while success and error response
  bodies are consumed, not only until response headers arrive.
• Treat page_size without current_page as Portkey's zero-based first prompt
  page so the requested bound and continuation metadata agree.
• Make Redis availability probes bounded and non-reconnecting, fail closed in
  CI, and retain local skips when a developer has no Redis service.
