Added

• The dialog provider messages accept refused, the text announced when a confirm is refused and the dialog stays open. The default is "The action was not completed. Review the dialog and try again." (#424)

Fixed

• Dialog and CommandDialog keep keyboard focus inside the dialog while busy: the confirm button stays focusable as aria-disabled and ignores activation, so it can no longer drop focus to the page or be submitted twice. After a refused confirm, such as a validation failure, focus returns to the confirm button (or to the title when confirm is disabled), the outcome is announced through a polite status region, and a held Enter or Space no longer submits again.
