Fixed

• The reference evaluator no longer fails a specification when a child projection event arrives before its parent instance: like Chronicle, it holds the child until the parent exists and then applies it with its original event values, at any nesting depth and across given, when and command commits; a child whose parent never arrives stays out of the read model, and an ambiguous parent still fails (part of #132)
• Removing a child, deferred or not, applies the ancestors' every mappings the way Chronicle does (part of #132)
