A Stage application menu and breadcrumb now follow the hierarchy the model declared - a group per module and per feature, nested as deep as the features nest - instead of a flat list of screen names.

Added

• A feature's contribute to Navigation entries carry the module and features they were declared in, and the Stage menu renders them as a nested tree: groups for the module and each feature, entries ordered by their order, each linking to the screen it opens with the current screen marked. The breadcrumb shows the groups leading to the current screen. A feature's entry arrives on each of its screens and is shown once. See Authored navigation hierarchy. (part of #237)
