Add query approval support for SecureQL connections:

• Detect when a SecureQL connection requires approval for certain query types as defined in SecureQL
• A new approval request and status panel to show the status of requests
• Let users request approval, check approval status, and pause or resume auto-checking
• When approved, users execute the query with a button click in the approval panel
• This also handles approved, expired, denied, failed, and cancelled approval requests
