What does this PR do? What is the motivation?

Improves documentation for Datadog Test Optimization library for Ruby:

  • marks start and end lines for tests as properly supported
  • removes outdated instructions in "manually instrumenting your tests"
  • adds proper "Manual instrumentation" instructions

* start documenting the environment variables for Ruby

* remove "set environment variables" step from manual instrumentation, expand configuration settings section a bit
