What does this PR do? What is the motivation?

Adds SQL Server 2025 to the DBM supported versions matrix and self-hosted setup page.

Changes:

  • setup_sql_server/_index.md: adds SQL Server 2025 row (self-hosted only), with note that cloud providers are in progress and Agent 7.79+ is required
  • setup_sql_server/selfhosted.md: adds 2025 to supported versions list with Agent version requirement

Merge instructions

Merge readiness:

  • Ready for merge

Additional notes

Some Cloud provider rows intentionally left blank pending validation (or availability). A follow-up PR will fill those in once confirmed.
