Adds SQL Server 2025 to the DBM supported versions matrix and self-hosted setup page.

setup_sql_server/_index.md: adds SQL Server 2025 row (self-hosted only), with note that cloud providers are in progress and Agent 7.79+ is requiredsetup_sql_server/selfhosted.md: adds 2025 to supported versions list with Agent version requirementMerge instructions
