Framing Gate (Always On)
Resolves the P1 design decision: serious framing issues (solution smuggling, zero metrics, scope mixing 3+ features) are now challenged automatically in one turn — no need to say "coach me." Minor issues get inline [flag: ...] tags. Full coaching remains opt-in.
