v0.24.2 — Manual MCP install section restored

Changed — README manual-install section restored and expanded

Feedback: collapsing "Manual install" under <details> in v0.20.2 was overcorrection. Users on Cursor / Codex / Cline / CI / team-shared configs had no visible answer to "how do I add Token Pilot as an MCP server?" — only the Claude-Code-specific npx token-pilot init path.

Restored as a full top-level section with per-client examples:

• Claude Code — both claude mcp add CLI shortcut and direct .mcp.json
• Cursor — .cursor/mcp.json stanza
• Codex CLI — ~/.codex/config.toml TOML block
• Cline (VS Code) — cline_mcp_settings.json
• Any MCP-compatible client — generic command + args
• Subagents install (Claude-Code-only) — scope flags + --force
• From source — for contributors / vendored installs

Also added an env-var reference table collecting the four env vars (TOKEN_PILOT_DENY_THRESHOLD, TOKEN_PILOT_ADAPTIVE_THRESHOLD, TOKEN_PILOT_BYPASS, TOKEN_PILOT_SKIP_POSTINSTALL) in one place — previously scattered across the codebase.
