fix: keep Cancel/OK visible and slim MCQ question guidance

Summary

• Cap non-danger question bodies with the same scrolled max-height as Confirm cards so tall referents cannot push Cancel/OK off-screen
• Drop the redundant “Text only — click or type (Audio off)” status banner (Audio checkbox is enough)
• Document short MCQ questions by default; put To/body or path+change in question only when confirming content

Test plan

• Open a tall non-danger MCQ (multi-line pattern-style body) and confirm Cancel/OK stay visible; body scrolls if needed
• With Audio off, confirm no gray text-only status banner above the options
• Short one-line MCQ still looks compact (no empty tall banner)
