Fix aggregateWithin continuation growth

Summary

• keep the idle aggregateWithin schedule recursion inside its innermost continuation
• install the never-ending and Done handling continuations once around the loop
• add a virtual-clock regression test that verifies idle schedule ticks do not grow the schedule fiber's continuation stack

Root cause

Each idle schedule tick recursively rebuilt the continuations for Effect.never and Pull.catchDone. Those continuations stayed retained until input arrived, so idle aggregateWithin and groupedWithin streams accumulated fiber stack entries indefinitely.

Validation

• nix develop -c pnpm vitest run packages/effect/test/Stream.test.ts (313 tests passed)
• nix develop -c pnpm --filter effect check
• nix develop -c pnpm lint
