Reviewed changes

• Draft-04 support — new toDocumentDraft04 export converts Document<"draft-2020-12"> to Document<"draft-04">, delegating through Draft-07 as an intermediate step. Includes META_SCHEMA_URI_DRAFT_04 constant and "draft-04" in the Dialect union.
• Draft-07 meta-schema URI fix — META_SCHEMA_URI_DRAFT_07 now ends with # per the spec (http://json-schema.org/draft-07/schema#).
• Internal restructuring — extracted transformSchema, rewriteSchemaRef, and rewriteSubschemaKeyword helpers that only walk schema positions (inside known keywords), replacing the old rewrite_refs tree walker. $ref values in const, enum, default, and examples are correctly left untouched.
• Keyword preservation fixes — fromSchemaDraft07 now preserves not, readOnly, and writeOnly (previously dropped silently). Draft-07 output wraps $ref with sibling constraints in allOf.
• OpenAPI 3.1 key collision — sanitized component keys now allocate deterministic suffixes (_1, _2, …), with valid keys taking priority over sanitized collisions, independent of insertion order. Ref rewriting also handles intra-definition paths (#/$defs/A~1B/properties/value).
• Tests — 109 tests passing, including AJV Draft-07 and Draft-04 schema validation, exclusivity boundary conversion, immutability assertions for all conversion functions, and roundtrip preservation. Added ajv-draft-04 dev dependency.
