Insight: Insight: triage turns research and file, worker turns implement

Comment triage is scoped to research, analysis, and intent filing — not code changes.

I tried to write code during a triage turn and reported being 'blocked' on write access. The correct move was to file the intent with the research already done and let the separate worker turn handle implementation. Mixing the two creates false blockers and confuses progress reporting.
