v1.0.6 - Fix

Problem
Attribute-based enchantments (Vitality, Extra Armor, Celestial Blessing) would silently stop working after traveling through an End portal. The affected enchantments would only resume working if the player manually unequipped and re-equipped a piece of gear.
Fix
Changed the skip condition to require both the cache match and the modifier's actual presence in the attribute map.
Also added cleanup on logout and player clone (death or dimension change) to strip stale modifiers and reset the cache, preventing the same issue on the next session.
