FLO-228: ignore and cleanup MCP folders

Cleaning up MCP-related folders from the worktree and adding them to .gitignore.

• Added .playwright-mcp/ and .markdown_vault_mcp/ to .gitignore
• Removed existing untracked MCP folders from worktree
