Physical iOS: every launch crashed before the BEAM printed a line
(MOB-199, regression from MOB-166 in 0.8.0). The generated SceneDelegate
calls mob_notify_window_connected() before mob_boot_runtime() on first
launch, and that notifier called enif_alloc_env() unconditionally; before
erl_start has initialised erts that jumps through a null allocator pointer,
which iOS reports as EXC_BAD_ACCESS at 0x0 with a CODESIGNING
"Invalid Page" termination and kills the app. The simulator happened not to
fault, and mix mob.deploy reported "Apps restarted" over a dead process. mob_notify_window_connected, mob_notify_color_scheme and mob_handle_back (the boot-splash edge swipe) now return early until nif_load has run (mob_runtime_up/0), and again after erl_start
returns. Affects apps generated by mob_new 0.4.33 and later against mob
0.8.0+; they pick the fix up on their next native build, no template
change needed.
