Brings Codex's skill registry up to parity with the other agents.
What changed
- Skill registry parity for Codex — the Codex SDD orchestrator now resolves and injects skills the same way the other agents do, so delegated phases load the right project standards.
- Permissions & SDD injection updated to support the registry wiring, with matching cleanup on uninstall.
- Docs refreshed —
README.md,docs/usage.md,docs/intended-usage.md, anddocs/skill-registry.mdupdated to reflect the parity.
