Fixed

• Plan text not committed to worktree branches — are plan created branches (are/plan/with-docs/<slug> and are/plan/without-docs/<slug>) but never committed the generated plans to them. Plans were only saved to .agents-reverse-engineer/plans/ on main. Now each branch receives a PLAN.md commit with the generated plan before worktree cleanup
