Health Gateway for Codex 1.0.2

Fixes an issue where several Codex connector processes could each open their own Health Gateway sign-in window. Authorization is now coordinated across processes: one process opens the secure sign-in, and all other connector instances wait for the same result. This release also prevents cancellation from cascading into additional windows and adds regression coverage for concurrent success and cancellation.
