• McpServerConfig::tools_list_changed lets servers advertise
  capabilities.tools.listChanged when their tool set can change after
  initialization.

• dump-version.sh now preserves handwritten Unreleased notes, can populate
  an empty release section from commits since the previous release, and refuses
  to overwrite populated release notes unless run with --force.

Fixed

• Destroy connection filter chains before the callback list they unregister
  from, preventing a deferred connection teardown from walking freed callback
  storage.
• Cover configurable tools_list_changed advertisement through the real
  initialize path so the default false and configured true cases cannot
  drift.
