dscode 0.0.14-alpha.12 fixes installation, update and session reliability problems.

• Preserve conflicting files when migrating legacy TUI directories.
• Reconcile the launcher, bridge, TUI and DSH runtime in one update transaction, including npx bootstrap and repairs.
• Skip unnecessary same-version reinstalls, honor automatic-update preferences, and preserve the update trigger.
• Isolate leaders by profile and TUI build, report the actual bridge version, and reject mismatches without restarting another session's leader.
• Verify the provenance and contents of reused runtime build dependencies.
• Keep tool approval pending until an answer or cancellation; a disconnect no longer appears as a user rejection.
