Performance release. On long sessions the out-of-band history pull was decoding ~6-7 MB of tool payloads per 50-message page (up to ~120 MB per activation). The panel now fetches its complete injected-document history once through a new /dir/injected-docs route that filters the durable log server-side (text blocks only); the docs stream is signature-gated, so the panel no longer re-renders per stream batch — only when the projected documents actually change.
