v0.12.3

Critical fix: the /dir/injected-docs route shipped since v0.11.2 called helper functions whose definitions never made it into the standalone package, so the route threw at runtime and the context tab degraded to live-window-only history. This restores the helpers and adds the revision-keyed server memo, plus the v0.12.2 per-session client cache.
