Changes to note

• aider: when the model's whole reply is an edit, the answer no longer comes out as just the file's name; it says what aider did, for example "Edited hello.txt.".
• A running session with no heartbeat on record is aged from when it was created or when its turn started, never from 1970, so the cleanup sweep cannot mark a task that just started as failed.
