gather report now requires a quote on every citation and asks the model again when one is missing. USAGE.md records the measured citation precision before and after the change; neither local model measured reaches a 0.80 lower bound.

• gather report now asks the model again when a citation has no quote, up to 2 more times, so one report can make up to 3 model calls. --quote-retries 0 restores one call. The prompt also forbids a bracketed excerpt number without a quotation right before it. The kept answer is never edited: if every answer has an unquoted citation, the last one is kept, its citation stays unchecked, and meta.quote_requirement reads unmet. Each call is recorded in meta.attempts, and meta.elapsed_s is now the sum over all calls.
