2026-04-17: Harden team-review-fix prompt layer (audit-driven)

  1. skills/team-review-fix/SKILL.md —
    • Step 3 reviewer spawn: added explicit SendMessage routing instruction, read-only constraint (no Edit/Write in Phase 1), and retraction discipline (re-read at HEAD before retracting)
    • Step 4 Phase 1: added architect ACK step after each TaskCreate; added team-lead fast-fail check that verifies every reviewer has at least one architect interaction before allowing convergence
    • Step 6a fixer spawn: added mandatory fixer discipline prompt covering task-ID discipline (use original IDs, TaskUpdate not TaskCreate) and pre-FIX_COMPLETE checklist (tests + lint + types on touched files)
    • Step 7 final report: expanded protocol compliance table with legend (✓/~/!), new rows for routing, read-only, ACK, task-ID discipline, fixer pre-flight, and orchestration intervention count
