Migrate ExternalData tag files to CIDv1-raw form. The CID blobs are added to ITKTestingData gh-pages in InsightSoftwareConsortium/ITKTestingData#44.
BoxheadCTFull.img.md5 points to a hash that 404s on gh-pages, Girder,
and itk.org. Comment out the 3 tests that depend on it
(TwoProjection2D3DRegistrationFullSizeCTTest, GetDRRSiddonJacobs-
RayTracingFullSizedCTTest{1,2}) and remove the now-unused
BoxheadCTFull.{img.md5,hdr.cid} tag pair.

Migrate ExternalData tag files in this module to CIDv1-raw form. Companion CID blobs added to ITKTestingData gh-pages in InsightSoftwareConsortium/ITKTestingData#44 — merge that first, then this.
