Eliminate unnecessary duplicate function definitions

These are functions that are defined on more than one file with different definitions. There may be a few places where this is necessary for bootstrapping, but it's a problem for lots of situations -- Masterscope, editdef etc pick the latest loaded or even worse...

Many of these are defined for D-machine page fault handler, which was done in Lisp(!).
There was a means of "locking" pages needed by the fault handler (which could not allocate any storage)
This is unneeded in Maiko .

It seems like there are several means of overriding the D-machine definitions.

1. depend on the runtime (MACHINETYPE)
2. add a pair \MAIKO.fn fn to \MAIKO.MOVDS
3. define the new version in MAIKOLOADUPFNS and override

only case 3 shows up as "same function".
