feat(agent): automation controls surface (dashboard + CLI + MCP)

Goal

The unified control surface: autonomy dial per action, audit feed, approval queue — across dashboard/CLI/MCP.

Deliverables

• Dashboard: new Automation tab (or extend ai-review-settings) — autonomy dial, audit feed, approval queue, kill-switch
• CLI: maintain status|pause|approve |set-level ; mine --repo X --auto
• MCP: control tools (read autonomy state, enqueue proposed action, surface approval queue)
