Both the TypeScript and Python servers speak the 2026-07-28 revision on
their official v2 SDK abstractions, share the identity postgres-mcp and
the version 1.1.0, and are described by a single root server.json covering
the npm and PyPI packages.

Fixes two defects present in npm 1.0.1: the entry point guarded startup
with a file:// URL comparison that never matched on Windows, so the server
exited without answering initialize; and it logged to stdout after
connecting the stdio transport, corrupting the JSON-RPC stream.
