fix(materialize): refuse nested output inside eng-init workspace

Agents derived --output from the intent name inside an init workspace
(e.g. reloj_checador/reloj_checador_escolar), orphaning bootstrap state.
The core now refuses new or empty subdirectories nested under a workspace
carrying .engineering/bootstrap.json and points at --output .; Pi and
OpenCode skills/commands carry the same hard rule. Bootstrap 1.0.3.
