Annotation UX fixes from live review feedback.

  • The selected line range stays highlighted while you type the comment (it previously collapsed to the cursor row until saved)
  • Inline comments word-wrap to the pane width with indented continuation rows — long text no longer runs off the edge
  • The comment/summary input bar keeps the end of long text visible (leading ellipsis)
