v0.2.14: fix MIDI mode blocked by phrase engage; show root note slider

- setPhraseEngage(false) each block in MIDI mode prevents phrase OLA
state from gating the shouldProcess check and silencing MIDI voices
- rootNoteSlider now visible and positioned in MIDI mode (was hidden at 0,0)
- PHRASE button disabled in MIDI mode (voices handle playback independently)
- ROOT label + note name shown flanking the slider
