Structural file reorganization

• Stepping away from the www folder
• Integration no longer copies files into /www/
• Logs moved from /www/esphome-update-manager/ to /esphome_update_manager/
• Frontend files now served directly from the integration folder
• Existing logs are automatically migrated
• Old Lovelace resource entries are automatically cleaned up
• No action required — upgrade is fully automatic
