Reliability fixes

• Retry the full PR diff when no completed review baseline exists; an unsuccessful first attempt must not become a zero-diff 5/5 rerun.
• Prefer provider-reported usage charges, discard model-invented runtime accounting, and retain known charges for malformed/empty/incomplete completions.
• Reject incomplete completions and emit bounded generation/finish/usage and parsing diagnostics without raw prompts or response content.
• Keep existing models and explicit retry policy. This release does not claim to eliminate model schema noncompliance or retroactively revalidate historical summaries.

Release verification passed regression tests, builds, provenance attestations, and execution checks on Linux X64, macOS ARM64, and macOS X64.

Rollout verification

• Action activation PR #125 merged as 8d37e8d0bc5f91e4ca855c45502b67290298c910; v0 resolves to that revision and pins runtime v0.9.5. Rust/site CI passed. The old released review engine was incomplete on the source and activation PRs; shipping used the explicitly approved review-gate waiver, not an empty-diff rerun.
• Homebrew update #26 merged as 76583238294dfd31d73279e1eb4f6340ea55b800; macOS install-and-upgrade CI passed old-version install, upgrade, and clean new-version install.
• Fresh consumer PR #14, created after moving v0, passed at exact head 1c0669d7ed24375cdfa6f9fb72739b0985e6a692; merged as 42a47664033d8813dbb9e994f4ca390200e83fc6.
• Consumer logs confirm Action SHA 8d37e8d0bc5f91e4ca855c45502b67290298c910, v0.9.5 attestation-gated binary installation, no source build, and 4.633-second startup (below 15 seconds, excluding queue).
• Both enabled angles passed 5/5 with no angle errors and 48 changed lines analyzed. Provider usage recorded two components, totaling $0.0099608. Existing smoke model openai/gpt-4.1-mini was unchanged; this is release verification, not proof that the production default model never violates the schema.
• The running placeholder and final canonical summary have the same comment ID, with exactly one summary and zero finding comments. No findings were returned, so this smoke did not exercise inline finding publication.
• Standalone v0.9.4-to-v0.9.5 upgrade and clean v0.9.5 install passed byte-for-byte comparisons with the released Linux archive.
