Add task tracking to the issue-plan command. Introduces Step 0 bootstrap with sequential task creation and a 7-row task table (Steps 0-6), absorbs the old Parse Input step into Step 0, adds state-transition markers to all steps, and updates CLAUDE.md sync bullets to reflect issue-plan as the 5th tracked command.

Part of #81. Fixes #93.
