Add due-diligence steps for evaluation outputs in commands. Inserts a "Gather PR context" step in pr-pre-merge, adds exploration lenses to issue-assessment, issue-plan-review, and issue-plan so evaluation outputs have corresponding evidence-gathering directives, and establishes the "Gather then Evaluate" development directive in CLAUDE.md.

Fixes #127.
