MedInsight

Problem Statement

Clinical documentation review and medical safety auditing are critical but time-intensive processes in healthcare. Manual review of clinical notes, prescriptions, and medical documents to identify potential safety issues, drug interactions, dosage errors, and guideline compliance violations requires significant clinical expertise and is prone to human error. Healthcare facilities process thousands of documents daily, making comprehensive manual review impractical. This creates a gap where critical safety issues may go undetected, potentially leading to adverse patient outcomes.

Success in this context means accurately identifying and flagging potential medical safety issues with high precision, enabling healthcare providers to make informed decisions quickly and reduce the risk of medication errors, adverse drug interactions, and guideline violations.

Objective

This project aims to develop an automated clinical notes auditing system that can analyze medical documents, extract relevant clinical information, and identify potential safety issues using natural language processing and machine learning techniques. The system should provide actionable insights with severity classification, enabling healthcare professionals to prioritize critical cases while maintaining high accuracy to minimize false positives that could lead to alert fatigue.

Constraints include the need for interpretable results, compliance with healthcare data privacy requirements, and the ability to process unstructured clinical text from various sources including PDFs and images.

Dataset

The system processes unstructured clinical text data extracted from medical documents including clinical notes, prescriptions, and lab reports. The data type is primarily unstructured text with embedded structured information (medications, dosages, symptoms, vital signs, lab values).

The knowledge base consists of curated medical guidelines and reference materials:

  • Drug dosage guidelines (5,000+ medication entries)
  • Drug interaction database (10,000+ interaction pairs)
  • Emergency symptom definitions (500+ red flag conditions)
  • Laboratory reference ranges (200+ test parameters)
  • Clinical treatment guidelines (comprehensive coverage across specialties)

Key features extracted from clinical text include:

  • Medications and dosages
  • Patient symptoms and complaints
  • Vital signs (blood pressure, heart rate, temperature, respiratory rate)
  • Laboratory values
  • Patient demographics (age, gender)
  • Medical history indicators

Data preprocessing steps include:

  • Text extraction from PDFs using pdfplumber
  • OCR processing for image-based documents using pytesseract
  • Text normalization and cleaning
  • Entity extraction and normalization (dosage units, frequency standardization)
  • Vector embedding generation for semantic search

Approach

The solution employs a multi-stage pipeline combining rule-based extraction, transformer-based named entity recognition, and a retrieval-augmented generation (RAG) system with specialized AI agents.

  1. Document ingestion and text extraction
  2. Named Entity Recognition (NER) using BERT-based models
  3. Entity normalization and standardization
  4. Semantic search against medical knowledge base using vector embeddings

Agent logic is rule-based with LLM enhancement for complex reasoning. No explicit model training is performed; the system leverages transfer learning from pre-trained medical and general language models.

  • False Positive Rate: Minimizing incorrect safety alerts to prevent alert fatigue

Why These Metrics: Entity extraction accuracy ensures reliable information extraction, which is foundational for all downstream analysis. Risk classification accuracy directly impacts clinical decision-making. False positive rate is critical in healthcare to maintain clinician trust and prevent alert fatigue. Response time ensures the system is practical for real-world clinical workflows.

Validation Strategy: The system is evaluated on synthetic test cases covering various medical emergencies (DKA, meningitis, myocardial infarction, stroke, sepsis) and common clinical scenarios. Each test case is manually validated by comparing system outputs against expected clinical responses. The test suite includes edge cases for dosage errors, drug interactions, missing tests, and guideline violations.

Entity Extraction Performance:

  • Drug extraction accuracy: ~92% on clinical text

Key Insights: The hybrid approach combining rule-based patterns with transformer models provides robust entity extraction, particularly for medical terminology. The multi-agent system effectively decomposes complex clinical reasoning into specialized tasks. RAG significantly improves the quality of recommendations by grounding responses in medical guidelines.

Limitations: The system requires curated knowledge bases and may miss rare drug interactions not present in the database. Performance depends on text quality from OCR, which can introduce errors in image-based documents. The current implementation processes documents sequentially and may require optimization for high-volume batch processing. Clinical validation on real patient data is needed for production deployment.

How to Run This Project

Step 1: Clone the repository

git clone <repository-url>
cd MedInsight

Step 2: Create and activate virtual environment

cd backend
python3 -m venv venv

Step 3: Install dependencies

pip install -r requirements.txt

Step 4: Set up MongoDB

Step 5: Build vector store

cd ../knowledge_base
source ../backend/venv/bin/activate

Step 6: Configure environment variables

Step 7: Run training / inference scripts

Start both backend and dashboard services:

This starts:

  • FastAPI backend on http://localhost:8000
  • Streamlit dashboard on http://localhost:8501
