Changes

bump(minor) feat!: single fast parser API

Replace the four parsers with parseCsv and parseCsvStream (breaking).
Both use the fastest line parser (22-27% faster after switching to
integer date math), locate columns by header name (supports Yahoo
Finance's Adj Close and reordered columns), and report skipped lines
with the failing field named.
Also: fetch functions accept any string | URL, added tests, CI
workflow, LICENSE, and module docs for a 100% JSR score.
