Fix heading navigation and active marks

  • With By section length or Equal shares tracking, clicking a mark now navigates to that mark's assigned scroll range, matching the active-mark rule and the settings preview.
  • Heading bookmarks opened through Obsidian and ordinary heading links use the same allocation in both reading and editing modes.
  • Follow the note and No tracking retain exact heading jumps.
  • Deferred navigation waits for rendering and source data, and is cancelled by newer navigation, manual input, or switching files.
  • Added regression coverage for rail clicks, bookmark navigation, late source loading, and navigation hook cleanup.
