feat: add bundled follow-up delivery

Summary

• add bundled follow-up generation and Telegram delivery plumbing, including a cron route for sending due follow-ups
• track initial follow-ups and reminder sends in core/db state, including migration coverage and persistence updates
• expand webhook, mutation, and follow-up tests and update current work documentation
