fix: stabilize MemOS trace ordering and scoring

Summary

• Improve MemOS trace capture, ordering, and idempotency for Hermes tool/subagent flows.
• Preserve scoring recovery and reward metadata so Memories and Tasks views show consistent status and scores.
• Align viewer ordering for grouped memory traces with task timelines, including subagent/delegate anchoring.

Test plan

• ruff check adapters tests/python && ruff format --check adapters tests/python
• npm run test -- tests/unit/pipeline/memory-core.test.ts
• npm run lint
• npm run build:all && npm pack
• Installed local tarball and restarted Hermes bridge/viewer/chat
