Patch release for upgrades from pre-0.3 managed configurations.

Fixes

  • Migrate legacy unmarked Codex MemoryGuard MCP sections before writing the managed section.
  • Prevent duplicate mcp_servers.memoryguard TOML tables during reinstall or upgrade.
  • Preserve unrelated Codex tables and standalone comments during migration.
  • Replace non-GBK status glyphs in memoryguard doctor with portable ASCII markers.

Validation

  • 381 tests passed.
  • Codex legacy migration reproduced before the fix and passes after it.
  • Windows doctor command and 0.3.1 wheel/sdist build verified.
